REGISTRATION IS OPEN FOR THE 2013/2014 SCHOOL YEAR

A full Registration Packet must be returned IN PERSON to the school office for review and acceptance before registration is complete.  If you currently reside out of the area, please call the school office at 623-247-2204 to make arrangements to mail Registration Packet and student records.  The following information must be included with Registration before enrollment will be considered:

State Issued Birth Certificate (hospital footprints will not be accepted)
Up To Date Immunization Records
Last Report Card (unless enrolling for Kindergarten)
If Applicable - Complete, Current Special Education IEP

If you are intending to register your child for the 2013/2014 school year, please look for the link titled "2013/2014 Registration Packet" on the sidebar to download and print Registration Packet, or come to school office to obtain a Registration Packet.  

If you are interested in more information about registering your child at Camelback Academy, please complete form below.  We cannot reply to the pre-registration form, so please include a phone number or email address for further correspondence. This form is for information requests only. 

KINDERGARTEN ENROLLMENT REQUIREMENTS 

Children must be 5 years old before September 1st, 2013 to qualify for Kindergarten enrollment.

Individual student evaluation for children born between September 1, 2008 - October 31, 2008 
will only be considered IF there are openings in Kindergarten as of July 12, 2013.  

CURRENTLY, THERE ARE NO OPENINGS IN KINDERGARTEN FOR THE 2012/2013 SCHOOL YEAR.
