A full Registration Packet must be returned IN PERSON to the school office for review and acceptance before registration is complete.  If you currently reside out of the area, please call the school office at 623-247-2204 to make arrangements to mail Registration Packet and student records.  The following information must be included with Registration before enrollment will be considered:

• State Issued Birth Certificate (hospital footprints will not be accepted)
• Social Security Card
• Up To Date Immunization Records
• Last Report Card (unless enrolling for Kindergarten)
• If Applicable - Complete, Current Special Education IEP

Kindergarten students must be 5 years old before September 1st, 2009 to qualify for Kindergarten enrollment.  CAMELBACK ACADEMY WILL NOT MAKE EXCEPTIONS TO THIS POLICY.
