School Year 2023-2024 Registration

Thank you for desiring to enroll your child at Camelback Academy, we look forward to working together in your child’s education.  Please read through the cover page on the form and complete your enrollment by returning this form in its entirety.  Completing the PRE-REGISTRATION FORM is the beginning of our registration process.  

IMPORTANT NOTE:  A completed Pre-Registration Form DOES NOT guarantee that we will have placement for your student. 

If we are at full enrollment or are full in a particular grade level, we will add students to a waiting list, at parent’s request. 

After you complete the Pre-Registration process, you will receive a link from our registrar to complete the enrollment process. Please use this helpful article for accessing and completing forms.

Kindergarten Enrollment Requirements

Children must be 5 years old by September 1st, 2023 to qualify for Kindergarten enrollment.
